American Health Care's disease management program is modeled after the nationally recognized Asheville Project where American Health Care provided clinical pharmacy support to trained Care Managers who provided face-to-face drug therapy and lifestyle management counseling to diabetic patients. With key Asheville Project leadership managing American Health Care's disease management program’s initiative, American Health Care developed its Improved-Asheville model of disease management, making significant improvements to the care management process, documentation, reporting and stakeholder accountability through use of our proprietary DOCS technology.
With the American Health Care Improved-Asheville disease management program your company will experience significant savings on medical costs, reduced absenteeism and greater productivity in the workplace. Published results demonstrate that the program minimizes patient complications, reduces hospital visits, and improves patient health significantly reducing employer health care costs. The program covers diabetes, high blood pressure, high cholesterol, asthma and 12 other chronic diseases. Lab and biometrics measurements (height, weight, blood pressure, blood sugar and cholesterol) are an integral part of the program together with a comprehensive Health Risk Assessment questionnaire. Using American Health Care’s Improved-Asheville care management model, medical and pharmacy data will be analyzed in order to identify employees that can most benefit from the program (typically 15% of the membership spends 80% of the medical cost). Once identified, American Health Care will provide recommendations on medical and pharmacy incentives for identified patients in order to encourage recruitment into the program and develop a communications and voluntary recruitment plan. Employees who choose to enroll in monthly visits with a certified and trained Care Manager (e.g., a pharmacist, nurse, nurse practitioner or certified diabetes educator). Care Managers in collaboration with, and in support of the primary care providers patient care plan, proactively manage the employee’s condition by ensuring that they take their medications, have appropriate lab tests performed and through managing progress towards lifestyle and diet goals. A key component of the program surrounds American Health Care’s DOCS technology, which allows the Care Manager to easily identify and manage gaps in the employee’s care versus evidence-based national guidelines.
